Enclosed you will find our registration form, health history, financial policy and privacy policy. Please complete and bring these with you to your first visit. If you have dental insurance, you will want to bring your insurance card or provide us with the name, address, phone number, and group and id numbers of your insurance company.

We have also enclosed a business card with an appointment reminder. Should you have any questions, please feel free to give us a call. We are looking forward to a relaxed and pleasant visit with you!
